A medical spa owner in Watford City, North Dakota, is facing multiple criminal charges after police discovered she was operating without any valid medical license.

Kendra Cain, owner of Royals Medical Spa, was arrested on Sunday after a detailed investigation revealed she had been illegally performing medical procedures, using a staff member’s license, and putting patients at serious risk.

How It All Started

The investigation began after a registered nurse, who had visited Royals Medical Spa as a patient, raised concerns about Cain’s actions. According to a report from the McKenzie County Farmer, the nurse noticed irregularities during her treatment and reported them to the Watford City Police Department. This tip led police to dig deeper into the spa’s operations.

Royals Medical Spa had opened in September 2025 and abruptly closed on January 22, 2026. Around that time, police received multiple complaints about the business.

Allegations Against Kendra Cain

Authorities allege that Cain had been:

  • Falsely presenting herself as a registered nurse
  • Using another staff member’s medical license to schedule patients, prescribe medications, and perform procedures
  • Performing treatments without any medical training or certification

Police confirmed that no medical or nursing license exists under Cain’s name.

What Police Found at the Spa

When police executed a search warrant at the spa on January 22, they discovered disturbing evidence in a patient treatment area, including:

  • A used IV bag, tubing, and needle
  • A syringe and open sharps container
  • Fresh blood on the floor near the IV setup

These findings suggested recent use of medical equipment by someone without proper credentials, increasing the risk of serious patient harm.

Patient Speaks Out

The registered nurse who reported the spa also told police that while receiving treatment, she had to “coach” Cain through inserting an IV. She said Cain:

  • Left the tourniquet on too long, causing her hand to turn purple
  • Allowed air to enter the IV tube, which can cause a potentially fatal air embolism

This firsthand account added weight to the police investigation and showed clear evidence of dangerous, unqualified practices.

Criminal Charges Filed

Cain was arrested and charged with:

  • Reckless endangerment
  • Unauthorized use of personal identifying information
  • Deceptive writings
  • Practicing nursing without certification

She was booked into the McKenzie County Correctional Facility but was later released. Her next court appearance is scheduled for March 26, 2026.

This case shines a spotlight on the growing number of unlicensed medical spa operators across the country. While med spas have become popular for offering cosmetic and wellness treatments, many of these services legally require trained and certified professionals.
